HUB Manager-Care Coordination

The HUB Manager of Care Coordination collaborates and coordinates with multidisciplinary teams to facilitate a safe quality experience for patients through the continuum of care. Liaison between all patient care services to ensure appropriate bed is assigned, patient care needs are met and treatment plans are executed in the appropriate level of the continuum. Recognizes and takes appropriate action in situations that require immediate intervention. Demonstrates technical and clinical knowledge; serves as a clinical resource and role model for staff.

Education: Registered Nurse with NYS License is required. BSN Preferred. PRI certification is also required.

Skills: Excellent communication skills both written and spoken. Organized and detail oriented. Ability to assess, evaluate, plan and coordinate patient placement activities. Serves as a positive and supportive resource to patients/families, and the entire care team. Demonstrates flexibility. Beginner proficiency of Outlook, Word and Excel is required. Beginner proficiency of Power Point is preferred.

Experience: Minimum 3 years in-patient acute care clinical experience required. Experience in care management preferred. Related specialty certification preferred. Two years progressive nursing leadership experience preferred.

Physical Requirements: Physical Requirements: Position is moderate and will require sitting for long periods of time interspersed with walking to and from various locations. Excellent verbal and listening skills required for communicating clearly and effectively with others. Ability to bend, stretch, reach, handle/finger delicate items required. Ability to carry files up to 5 lbs. Reasonable accommodation will be made to enable individuals with disabilities to perform the essential physical demands.
