Good Shepherd Hospice is one of the largest hospice & palliative care providers on Long Island New York, caring for patients and families with serious and terminal illnesses in community and hospital settings. In the role of hospice physician at Good Shepherd Hospice, you will work collaboratively with the Medical Director and other hospice and palliative medicine physicians.
Responsibilities will include:
Maintaining oversight of the day to day medical care of Good Shepherd Hospice patients on assigned teams, including certification/recertification/decertification determinations.
Guiding the hospice interdisciplinary team in the formation of a plan of care to meet patient/family needs.
Providing home visits as necessary to address complex symptom management needs, serve as a patient's attending physician, and perform required face to face evaluations.
Providing consultation as part of the Hospice and Palliative Care interdisciplinary team in the inpatient/outpatient setting as assigned.
Educating a variety of learners, including students, residents, fellows, and colleagues in medicine, nursing, & other disciplines about the principles of hospice & palliative care.
Participating in a physician on call schedule to assure hospice physician coverage 24 hours a day, 7 days a week.
The successful candidate will be a graduate from an accredited school of medicine or possess ECFMG Certification. Primary board certification is required; certification in internal medicine or family medicine preferred.The candidate should be BC/BE in Hospice and Palliative Medicine or possess/be eligible for Hospice Medical Director Certification. A minimum of two years of postgraduate direct patient care is required. Commitment to hospice & palliative medicine principles of care, along with knowledge and skills in patient assessment, pain and symptom control, psychological aspects of terminal illness, and pharmacotherapeutics will be expected.
