The Hospice & Palliative Medicine Physician works collaboratively with the Medical Director and other hospice and palliative medicine physicians and is responsible for the medical care for the patients on the Good Shepherd Hospice program and the provision of palliative care consultation services at CHS facilities.

Minimum Requirements:

Current license to practice medicine in NYS

Current controlled substance registration certificate

Graduate from an accredited school of medicine or ECFMG certification

Board certification in primary medical specialty, internal medicine or family medicine preferred

Advanced certification in Hospice and Palliative Medicine preferred

Eligibility for Board Certification in Hospice and Palliative Care or Hospice Medical Director Certification required

Minimum two years postgraduate direct patient care in the absence of fellowship training in hospice & palliative medicine

Familiarity with and commitment to the hospice and palliative medicine principles of care for patients with serious illness
