Catholic Health Home Care manages home care services for approximately 3,000 patients daily across Long Island and Queens. Specialized services include skilled nursing care, physical and occupational therapies, medical social work, maternal-child and pediatric services, home health aide assistance, nutritional counseling, pastoral care, infusion services, telehealth, palliative care, wound care and behavioral health services.
The Nurse Mentor provides instruction, guidance and support to new graduate nurses.This important role is responsible for field visit demonstration and evaluation, support in the development of critical thinking skills, and providing meaningful feedback to facilitate safe and effective patient care. The mentor will encourage the new nurse to grow into their role and be successful in completing the residency program. Provides ongoing evaluation of the quality and effectiveness of the educational activities to ensure that they maintain and enhance professional development that promotes the delivery of safe, high quality care. Recommends programs or other methods of education to meet identified needs and promote staff safety, patient safety and high quality care.Ensures that educational activities are congruent with the agency's mission, priorities, and resources.
Position requirements include:
- Bachelor's Degree in Nursing required. Master's Degree preferred.
- License and current registration to practice as a Registered Nurse in the state of New York.
- Minimum of three (3) years' experience in a Certified Home Health Agency (CHHA) required.
- Good computer skills, including Electronic Medical Records.
- Strong communication skills and demonstrated ability to develop and deliver presentations to a large group of staff.
