Nurse Liasion-Admissions

The Rehab Admissions Nurse Liaison will screen patients with rehab diagnoses on-site at referring Long Island hospitals for admission to St. Charles IP Rehab. Duties include identification of candidates who meet program criteria, completion of a pre-admission screen, education of patients/ family members, determination of medical stability and readiness for rehab program, and coordination of a smooth transition from the referring facility to the rehab setting. The ideal candidate will have great communication skills, the ability to function independently and exhibit professional behavior as a representative of St. Charles Hospital. NY State RN license & registration; Valid NYS Drivers License and BLS certification required. 5yrs+ of clinical hospital experience required, rehab experience preferred.
