Manager-Care Coordination-HUB

Graduate of an accredited school of nursing; Graduate of Bachelor's degree in nursing preferred.

Two to three years' experience in health care related discipline (hospital, agency, etc.) and/ or
previous discharge planning experience and/or community health care experience preferred; must be extremely motivated, organized, flexible, and detail oriented. Excellent interpersonal skills (oral and written).

Ability to respond to short deadlines.

Effective problem solving abilities.


NYS RN license required; PRI and/or Screen certification preferred.

Job Responsibilities:

Uses clinical knowledge to plan and coordinate system?wide patient placement activities.

Appropriately escalates any cases not meeting admission criteria to Director of Clinical Care Coordination.

Collaborates with HUB Assistant to consider nursing staff capacity in planning patient placement.

Assists the Service Organizer with priorities regarding patient flow to ancillary testing and procedure areas.

Serves as a consultant and educator with medical staff regarding patient status, patient placement, payor requirements, and utilization of resources.

Obtains clinical information for direct admits.

Creates and tracks direct admission and hospital transfer cases in conjunction with the HUB Assistant.

Evaluates appropriateness of admission (patient status, level of care) for direct admits and facility transfers.

Determines DRG?based initial LOS values for direct admits and facility transfers (all status/types) and enters into Epic.

Anticipates and facilitates accommodations for care needs of patients being admitted to the nursing unit (patient requiring a bariatric bed, special equipment, language line, etc.).

Evaluates bed requests and assigns bed to facilitate flow.

Serves as point of contact for coordinating internal facility transfers (i.e. Rehab to Acute, Acute to psych).

Monitoring Bed Requests from Cath Lab and other procedural areas for post procedure admissions. Assigns bed and status prior to admission.

Float to other units to provide coverage when necessary.
