Facilitates patient care during hospitalization of an assigned number of cases by coordinating, facilitating and monitoring resources to achieve optimal clinical outcomes, appropriate utilization of services and financial goals. Develop and execute in a timely fashion a discharge plan that is consistent with the identified needs of the patient.
Two to three years' experience in health care related discipline (hospital, agency, etc.) and/or previous discharge planning experience and /or community health care experience preferred.
Completes psychosocial assessments collaborating with ED MCC and care team for management and intervention (i.e.) substance abuse, guardianship, domestic violence, homelessness, etc.
Works collaboratively with ED staff, Physicians, MLP's, and MCC to identify patients requiring Social Work intervention.
Calls/conversations with physicians regarding discharge planning needs.
Collaborate with members of the health care team to achieve a cohesive and comprehensive discharge plan for patient.
Assesses patient's functional status, cognitive ability, support systems, available resources, and barriers to care to identify post ED discharge needs.
Receives referrals from ED MCC for support with social issues and patients not meeting criteria for admission who require community resources.
Coordinate medication assistance as requested.
Keeps ED MCC informed of status, interventions, and plans to progress patient's discharge from ED.
Works collaboratively with ED care team to coordinate patient's from ED.
Provides community resources information as requested and facilitates or completes referrals as directed.
Coordinates patient-family conferences; assists with management/support of patient/family situations.
Helps family members and patients that are in need of further conversation regarding advance directives.
Provides information, education, and resource planning for patients and/or families in crisis.
Contacts local community based agencies as needed including CPS, DSS and APS.
Provide emotional support to patients and family members in crisis.
Documents interventions in the EMR.
