Develop and execute in a timely fashion, a safe plan that is consistent with the identified needs of the patient. Responsibilities: Demonstrates consistent ability to perform clinical social work skills necessary in the care and treatment of patients receiving medical outpatient follow up including addressing# mental health concerns.# Devotes attention and resources to provide the highest quality patient experience. Perform thorough needs based psychosocial assessments, within the required timeframe, for expectant mothers, patients interested in pursuing bariatric surgeries, and other family care center patients. Provide follow up with expectant mothers throughout their pregnancy in order to address any psychosocial barrier identified that could impact the health of the mother/pregnancy. (mental health, substance abuse, domestic violence, housing).# During the third trimester, complete an assessment of expectant mother#s readiness for care of the baby in order to address any concrete needs and safety concerns. Provide crisis intervention as needed to any Family Care Center patient due to various factors including illness, mental health, substance abuse, miscarriage, housing, domestic violence.# Complete assessment tools (PHQ-9 and GAD-7) in order to identify risk factors and address issues such as suicidality. Provide brief counseling and goal oriented therapy with linkage to ongoing community mental health treatment. Initiates follow up outreach to no show or canceled appointments in order to identify and address barriers to treatment and provide continuity of care.# Foster a collaborative relationship with the medical staff and other members of the health care team to provide quality patient care. Daily contact to physicians and other members of the healthcare team regarding medical needs in order to develop a safe and comprehensive plan of care including addressing any mental health needs and access to concrete services. Develop treatment plans that are responsive to patient/ family concerns, health care status, available resources, age-specific problems and diagnosis specific issues. Develop treatment plans to ensure access to care, refer to community based agencies (ie. transportation, Motivcare,and Department of Social Services.) Provide appropriate referrals for patient/family to various resources. (housing, home care, layette, newborn furniture, substance abuse programs, mental health agencies, and Child Protective Services.) Document all interventions in a timely and accurate manner using the EMR. Attend and participate in staff meetings/in-services in order to identify any areas for performance improvement and awareness of resources to benefit Family Care Center patients. Demonstrates an awareness of and respect for the cultural diversity of patients and co-workers by treating all people with fairness and respect.# Qualifications: Education:#Graduate of an accredited school of social work; Graduate of Master#s degree in social work required. Experience/skills: Two to three years# experience in health care related discipline (hospital, mental health agency, etc.) and/ or previous discharge planning experience and/or community health/mental health care experience preferred. Must be extremely motivated, organized, flexible, and detail oriented; Excellent interpersonal skills (oral and written). Ability to respond to short deadlines. Effective problem solving abilities. Bilinqual - Spanish preferred Licensure: LMSW by the State of New York required.
