St. Francis Hospital is dedicated to providing a supportive environment, committed to the highest standards of patient care, where health care professionals can develop their expertise and strengthen their credentials.# Registered Dietitian Community Health Requirements: Education # Master#s Degree in Nutrition preferred, Bachelor#s Degree in Nutrition # Dietetics required Licensure # Registered Dietician in New York State required Physical Demands # Must be able to speak, teach individuals and groups, ability to prepare food demonstrations Computer Skills # Word, Power Point, Excel Experience # Clinical background with patients preferred. Patient teaching/counseling experience 1 # 2 years recommended. Public speaking experience preferred. Experience with electronic medical record required. # Registered Dietitian Community Health Responsibilities: Provide medical nutrition therapy to Cardiac Rehab, Pulmonary Rehab, community patients as well as physician referrals Conducts individual nutritional counseling based on the individual#s clinical condition, age, lab values, and specific needs Able to prepare individualized meal plans for patients based on BMI, calorie, protein, fat and fluid calculations Provides individualized Weight Management Counseling to Patients Teaches monthly #Heart of Good Eating# class and participates in other community wellness programs and health fairs to groups Answers questions and assists with scheduling patients to consults and programs that fit their needs Serves as information source on nutrition to staff members Participates in Performance Improvement Provides Nutritional counseling to Outreach patients as needed Actively assists with marketing activities to promote nutritional services Prepares nutrition presentations and classes inclusive of power point slides Records monthly statistics Assists with Diabetes Prevention Program Prepares materials/handouts and participates in health fairs, World Stroke Day, Diabetes awareness month, National Nutrition month, and other special programs Participates in offsite community health and wellness programs as SFH representative as needed # # At Catholic Health#your well-being comes first, with comprehensive compensation and benefits; our offerings go beyond the basics.
