DUTIES: Coordinates a patient-focused team in providing direct and collaborative health care in a variety of clinical settings designed to promote and restore optimal health across the life span. Utilizes all components of the nursing process in providing care that reflects an understanding of individual cultural and religious diversities and needs, and recognition of patient rights. Utilizes knowledge and expertise in case management to provide comprehensive, coordinated, seamless care to individuals and aggregate groups. Coordinates, integrates and facilitates the patient/provider relationship along the continuum of care. QUALIFICATIONS: Graduate of an accredited school of nursing. Bachelors Degree preferred. Current licensure as R.N. in New York State. ANA certification or certification from appropriate agency in specialty required or must be working toward certification. One year med/surg experience and one year specialty experience preferred. ACLS, BLS and NRP certification required. SPECIAL REQUIREMENTS: Ability to operate all equipment found in care center as necessary to perform all prescribed duties. Ability to access and navigate clinical computer system. Ability to communicate in a tactful, diplomatic way. Ability to initiate projects and to carry out day to day operations with a minimum of supervision. Ability to handle information with strict confidentiality. Excellent written and verbal communication skills.
