Care Manager

Responsible for assessing, planning, implementing, monitoring and evaluating options and services to effect an appropriate individualized plan of care for patients across the continuum; performs required activities for Utilization Management, Transition Planning and identification of actual or potential quality/risk indicators in compliance with regulations and organizational goals to ensure optimal patient outcomes and length of stay efficiency; considers payer requirements and responds accordingly to avoid adverse financial consequences to patient and hospital.

Must be a graduate of an Accredited School of Nursing. RN with current NYS License required. Bachelor of Science in Nursing preferred. Minimum of 3 years experience in an acute care setting required. CM (Care Management), UM (Utilization Management), TP (Transition Planning-discharge planning), Managed Care or Home Care experience preferred. Computer and typing skills required.

BLS/AED training required and accepted from only 2 organizations:

1. American Heart Association - BLS/ACLS for Healthcare Providers (CPR & AED) Program

2. American Red Cross - CPR/AED for the Professional Rescuer

No online programs from any organization will be accepted.
