Manager Care Coordination

Supports GSHMC AIM 4 EXCELLENCE by ensuring all decisions and actions ensure "Patients are the priority and Excellence is our goal".The patient is at the center of our care model as we seek to provide a safe quality experience for our patients through the continuum of care.Collaborates and coordinates with multidisciplinary teams to facilitate a safe quality experience for patients through the continuum of care.Liaison between all patient care services to ensure appropriate bed is assigned, patient care needs are met and treatment plans are executed in the appropriate level of the continuum.Demonstrates technical and clinical knowledge; serves as a clinical resource and role model for staff.Maintains current knowledge of health care economics, trends and reimbursement methodologies and applies knowledge to daily practice. Recognizes situations that require immediate intervention. Continues to develop professionally and apply knowledge and skills necessary to provide care appropriate to each patient.

BSN Required (Applicants currently enrolled in a BSN program may apply).Knowledge of Outlook, Word, Excel, Database and Power Point required; must be flexible and able to work in stressful environment.Minimum 3 years clinical experience required.Two years leadership / experience in care management preferred.
