Overview

Catholic Health Home Care is looking for passionate, dedicated LPNs to provide comprehensive care to patients at home.

 Team 7 Covers Northern Suffolk; Centereach, Head of the Harbor, Lake Grove, Nesconset, Old Field, Port Jefferson, Port Jefferson Station, Setauket, South Setauket, St. James, Stony Brook, and Terryville.

    • 5x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends

Job Details

Our Home Care LPN’s:

    • Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
    • Measure vital signs, Administer IVs, change dressings, clean wounds as needed
    • Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
    • Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
    • Provide patient and family education for disease management and health promotion s directed by physician’s plan of treatment.

Minimum Qualifications:

  • New York State Licensed Practical Nurse (LPN) license
  • Minimum of one year of recent hands on experience
  • Basic Life Support Certification


Salary Range

USD $32.00 - USD $40.00 /Hr.
