Catholic Health Care at Home demonstrates our commitment to serving patients where they are, delivering skilled, compassionate care in the setting that best meets their needs. Under the Care at Home umbrella, we provide three core programs: Home Care, Palliative Care, and Good Shepherd Hospice.

By becoming part of the Care at Home team, you will work alongside compassionate, skilled clinicians to deliver high-quality, patient-centered care to the communities we serve.

Home Care

Catholic Health’s certified home health agency is accredited by the Joint Commission and designated as a Home Care Elite agency. Patients who need home health services are in the caring and capable hands of expertly skilled Nurses, Physical Therapists, Occupational Therapists, Medical Social Workers and more. In addition, we are proud to also provide specialty services, including but not limited to, behavioral health services, pediatric care, and telehealth services. Each patient’s individualized plan of care is designed by the health care team in conjunction with the patient, family and the physician.

Founded by the Congregation of the Infant Jesus in 1905, Catholic Health’s Home Care branch continues the Sisters' mission to provide care to the sick without regard for a person’s religion, race, gender or economic status.

Good Shepherd Hospice

Good Shepherd Hospice provides comprehensive, compassionate care to people whose life expectancy is limited by serious illness. Accredited by the Joint Commission, our team of board-certified physicians, nurses, social workers and spiritual care providers deliver expert medical, emotional and spiritual care that focus on quality of life and symptom management.

Palliative Care

Catholic Health’s Palliative Care program provides specialized, patient-centered care for individuals living with serious illness, with a focus on improving quality of life through expert symptom management and supportive care. As part of our integrated Care at Home program, our interdisciplinary team—including physicians, advanced practice providers, nurses, and social workers—partners closely with patients, families, and providers across home, hospital, and outpatient settings to manage complex symptoms, support care planning, and adapt care as needs evolve.

Home Care and Good Shepherd Hospice hire Registered Nurses and Licensed Practice Nurses who strike the right balance of being expertly skilled, empathetic, and dedicated. Our nurses have the unique experience of caring for patients in their most vulnerable times, whether that be providing 1-to-1 care to patients in their home or supporting a patient and their loved ones in their last moments of life.

Nursing in Home Health & Hospice can provide you a much different work experience as you are not confined to a specific facility that you report to daily. You have the ability to be flexible with your day and be autonomous in your work. You will have unlimited support from both your leadership team, as well as the interdisciplinary and clinical teams on a daily basis to ensure you have what you need to provide optimal patient care and well-being.

Our providers at Home Care and Good Shepherd Hospice are comprised of Physicians and Nurse Practitioners. In addition to our hospice care both inpatient and in the home, our providers are onsite at our acute care facilities providing palliative care. No matter the environment, we ensure we are bringing on providers that are extremely knowledgeable regarding the disease process, compassionate by nature, and who encompass our I-CARE values.

Working as a provider in the Home Health & Hospice Teams at Catholic Health provides a different perspective as we are faith based and focused on serving patients physically, but also spiritually and emotionally.

Our Allied Health team at Home Care and Good Shepherd Hospice is able to provide services and care to patients in the comfort of their own home. Whether you are a Therapist ensuring a patient can get around their home after a major surgery or speak and function after a stroke, or a dietician assisting a patient with learning about a new diabetic diet, you are making a difference and evaluating progress in a very substantial way.

At Home Care and Good Shepherd Hospice, our Allied Health team includes but is not limited to Physical Therapists/Assistants, Occupational Therapists, Speech Language Pathologists and Dieticians. Being that you are not working specifically in one location, you will have the ability to be flexible with your day and be autonomous in your work.

Our Home Health Aides at Home Care and Good Shepherd Hospice play a truly special role in a patients care. While providing medical care is imperative, all patients need personal care to remain at home – this includes supporting activities of daily living such as a hot meal, assistance with bathing , help with mobility and they need to be comfortable, whatever that means to them. Our Home Health Aides enable patients to stay home and live as independently as they potentially can. They build relationships with the patients and their family members, ensuring the patient is able to complete their activities of daily living.

Working in the Home Health & Hospice Teams at Catholic Health allows for flexible schedules and for career growth. Many of our Home Health Aides start in this role and continue their education, utilizing tuition reimbursement to obtain their LPN or RN licenses to provide even further care for our patient population.

Our Social Work & Spiritual Care teams at Home Care and Good Shepherd Hospice provide services beyond medical care. They ensure that patients receive the care they need physically, spiritually, and emotionally in the comfort of their own home.

Being a part of our Social Work & Spiritual Care departments is extremely rewarding. Our Social Workers provide organization, resources, and assurance as patients and family members navigate through different process. On a more spiritual level, our Chaplains provide a sense of hope, peace, and dignity to patients and family members when they need it most.

Our Leadership & Business Support roles at Home Care and Good Shepherd Hospice play a unique role in our integrative patient care model. These are the roles that may not necessarily always be out in the field providing patient care, but supporting the clinicians and employees who do so.

Our Leadership roles vary across our organization as typically they are specialized in their area of practice or discipline. We empower our leadership to do what they do best – lead their teams. We ensure that our leaders have the resources to support our caregivers in their work every day. Our Business Support roles range from Business Operations Associates to Team Manager Assistants. Although not in the home or at the bedside, they are imperative to providing the complex care needed by the patients we serve.
